Braces vs. Invisalign: Which Is Best for Me? 10 Questions Answered

There is no perfect appliance. Braces, Invisalign and hidden lingual braces each have real strengths and real trade-offs. The right choice depends on your bite, your habits and your life during treatment.

The biggest difference comes down to one word: compliance. Aligners only work when you wear them. Braces work whether you remember them or not.

This guide is written by Drs. Bob and Juliana Miller of Miller Orthodontics in Park City and Heber Valley. Dr. Bob Miller has used Invisalign since 1999 and co-authored one of the first Invisalign case-report articles, published in the Journal of the American Dental Association in 2003. Dr. Juliana Miller trained at the University of the Pacific, where Invisalign was incubated. Together we’ve treated more than 3,000 aligner cases and 500+ Lightforce custom-braces cases, and we offer InSmile lingual braces too. Because we use all three, we have no reason to steer you toward just one.

1. What’s the difference between “compliance-driven” and “compliance-free” treatment?

Aligners are compliance-driven: they move teeth only while they’re in your mouth. Braces, including Lightforce and InSmile, are compliance-free: they’re bonded in place and apply force 24 hours a day, 7 days a week.

That doesn’t make one better. It means the question isn’t just “Which works best?” It’s “Which works best for me, given how I’ll actually live for the next 12–24 months?”

2. When are braces more predictable than aligners?

Braces win when a tooth needs a firm, constant push in a direction plastic struggles to grip. In our experience, two movements top the list:

  • Rotating long-rooted teeth, especially canines. Canines have the longest roots in the mouth and a rounded shape that aligners tend to slip around. A bracket holds the tooth and turns it steadily, day and night.
  • Moving teeth up or down (intrusion and extrusion). Pulling a short tooth down into the bite, or pushing an over-erupted tooth up, takes continuous force. Braces deliver it 24/7. Aligners only deliver it during the hours they’re worn.

Aligners can still treat many of these cases. They’re simply less predictable, which can mean more refinements and a longer finish.

3. How many hours a day do aligners really need to be worn?

We ask for 20–22 hours a day. That’s the minimum needed for predictable tooth movement, not a nice-to-have.

In practice, aligners come out only to eat, brush and floss, and they go back in right after. Leave them in a lunch bag through a ski afternoon, and the teeth fall behind the plan. The next aligner won’t fit, and the case drifts off track.

4. What happens when teens burn out on aligners?

It’s common. In our experience, the typical teen burns out after about six months of aligner wear. The novelty fades, the cases get left on the nightstand, and the hours drop.

That’s why we watch teen aligner cases closely. We track progress through Dental Monitoring, an at-home scanning app, plus regular office visits. Catching a slip early means fewer refinements and fewer “wrong” tooth movements caused by missed wear.

Parents should ask any orthodontist: How will you know if my teen stops wearing them, and how soon?

5. Who is better off with aligners they can take out?

For some patients, being able to remove the appliance is a real advantage:

  • Adults with crowns, veneers or bridges. Bonding brackets to porcelain and other restorations is harder and can risk the restoration. Aligners avoid that.
  • Patients with special cleaning needs. If you have gum disease or need careful daily cleaning, taking aligners out to brush and floss normally keeps gums healthier.
  • Performance athletes. Figure skaters, dancers and others in non-contact, on-stage sports can remove aligners for competitions and photos.

6. What are InSmile lingual braces, and why are they unique?

InSmile braces are bonded to the back of the teeth, so they’re truly invisible. They combine the best of both worlds: hidden like aligners, but working full-time like braces.

  • Truly invisible. Nothing on the front of your teeth, and no plastic shine in photos.
  • Constant force, even while eating. A custom-programmed Smartwire works 24/7. That makes InSmile precise for harder cases: rotated front teeth and canines, and arches that need expansion.
  • Compliance-free. There’s nothing to remember to put back in.
  • Normal flossing. The Smartwire has built-in loops between the teeth, so you can floss conventionally. That’s something traditional braces don’t allow.

7. What’s the catch with lingual braces?

The main trade-off is an adjustment period. Because the appliance sits where your tongue rests, speech and tongue comfort take time to settle, sometimes up to two weeks. Teens usually adapt faster than adults.

InSmile’s design has been refined with the tongue in mind. Its smooth surfaces help the tongue adapt quickly.

8. Aren’t all braces the same? Where do Lightforce braces fit?

No. Lightforce brackets are 3D-printed and custom-designed for each tooth. The key is the bracket slot, the groove the wire sits in. Slot position determines where each tooth ends up.

When the slot is placed precisely from the start, less time goes into fine-tuning at the end. In some cases treatment time drops 30–50%, which means fewer wire-adjustment visits.

Lightforce now comes in clear or metal, so you get the 24/7 predictability of braces with a low-profile look if you want it.

9. Can I switch systems midway, and how different is daily life?

You can switch, but it costs extra, because each system carries its own lab fees. That’s the best reason to choose an orthodontist who offers all three digital systems and has deep experience in each. The right choice up front saves time and money.

Day to day, the main differences are food and visits:

  • Food: Our rule for any brace system is simple: if you can’t stick a fork in it, avoid it or cut it into small pieces. Aligners come out to eat, which is a real plus, but they must go right back in after. Ask yourself honestly: will that fit my lifestyle?
  • Starting and visits: In-house 3D printing allows same-day starts. Dental Monitoring lets us check progress from home, so you need fewer office visits on any system.

10. So how do I decide what’s best for me?

Start with honest questions at home, then bring them to your consult:

  • Teens: What do most of your friends have? Peer comfort matters, and so does an honest answer about 22-hour wear.
  • Parents: How were you treated, and how did it go? Your experience shapes what you expect.
  • Adults: More of our adults choose aligners than InSmile. We make sure they understand the compliance demands before they start.

All of these opinions weigh in, alongside what your bite actually needs.

The bottom line

At your consultation, our team takes complete records: x-rays, photos, a digital scan and a virtual preview of your new smile. Then Dr. Bob or Dr. Juliana walks you through which of the three systems fit your bite and your lifestyle. You decide together with your doctor, with no pressure.

The best appliance is the one that fits your smile and your life during the process.
